Repeat Prescriptions

You can order your repeat prescription in a number of ways:

  • Ordering online.
  • By bringing your written request into the surgery
  • Alternatively please post the right hand side of your repeat prescription in the box outside dispensary. When the surgery is closed please post in the letter box located by the side door on the wall.
  • By emailing your request to culverhay-berkeley.gp@nhs.net. Please ensure you include your name, address and date of birth with your request

We cannot take requests over the phone. 

Please allow 2 working days, excluding weekends and Bank Holidays, for your request to be processed.

Medication Reviews

Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip.

You can request a medication review by completing our online form.

Dispensing

We are able to dispense medications to all our patients who live more than one mile from a chemist. We are not able to dispense to the local community but we can arrange for your prescription to be delivered to the local chemist.

We provide a medication delivery service for patients aged 65 and over and for the elderly or infirm in the following villages. When you order your repeat prescription, please let the dispenser know if you would like to use the free delivery service.

You can order your repeat prescription in a number of ways:

  • By posting the right hand side of your repeat prescription in the box in reception
  • By emailing your request to culverhay-berkeley.gp@nhs.net. Please ensure you include your name, address and date of birth with your request

Unless you tell us otherwise we will send all prescriptions to the Chemist in Berkeley.

Please allow 3 working days, excluding weekends and Bank Holidays, for your request to be processed.
