Welcome to The Culverhay Surgery

We are a group practice providing traditional family medicine to a high standard in our own purpose-built surgery. Our website has been designed to provide clear and concise help and give our patients the information they require in an easy and convenient format. It has been built with the patient’s needs at the forefront of everything, from checking surgery times to letting us know what you think of us. The technology also means you can now do a lot of things from the comfort of your home such as order a repeat prescription or booking an appointment.

Summary Care Record - Important information

The Culverhay Surgery is planning an upload of summary care records on the 19th August 2014 for all of their patients who have not opted out of this scheme. This is an initiative introduced by NHS England to produce an electronic record called the Summary Care Record (SCR), which will be used to support your emergency care.  A Summary Care Record will contain information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed or when you are away from home in another part of England.

To find out more please download the attached leaflet HERE or follow this link to the NHS Summary Care Records website.

Once you have read and understood the information about SCR and if you wish to opt-out, please download the Opt-out Form below and return to the practice.

Care Data

your care data

NHS England is introducing a new, modern data system for the NHS in England. The system will be known as, and its purpose is to provide timely, accurate information to citizens, clinicians and commissioners about the treatments and care provided by the NHS.

Information about you and the care you receive is shared, in a secure system, by healthcare staff to support your treatment and care.

It is important that the NHS, can use this information to plan and improve services for all patients. It would link information from all the different places where you receive care, such as your GP, hospital and community service, to help provide a full picture. This will allow theNHS to compare the care you received in one area against the care you received in another, so they can see what has worked best.

You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening you will find more information regarding this subject by following this link.
