If you are over 65, or pregnant, or in an at risk group you will be eligible for a NHS flu vaccination, please contact the surgery as soon as possible to book in for a flu vaccine. The flu clinics will be held:

Friends and Family Test
The NHS has asked all GP surgeries to provide patients with the opportunity to provide feedback, following their treatment. This is known as the Friends and Family Test (FFT).
It has to be at least two questions, the first being a closed question “…would you recommend…. To your friends and family..” to which the answers vary from “Extremely likely” through to “Extremely unlikely”. This is then followed by a request to leave a comment as to their reasons.
By 1st December 2014, all GP practices must offer these two questions to patients, following every episode of treatment. They then need to collate and analyse the data, report the summaries to NHS England and then look at the comments for common themes to make service improvements. NHS England will then publish the FFT score on a national website that is public.

Summary Care Record - Important information
The Culverhay Surgery is planning an upload of summary care records on the 02nd Septemeber 2014 for all of their patients who have not opted out of this scheme. This is an initiative introduced by NHS England to produce an electronic record called the Summary Care Record (SCR), which will be used to support your emergency care. A Summary Care Record will contain information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.
Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed or when you are away from home in another part of England.

Patients of the Culverhay Surgery can now book, cancel and rebook appointments online, and also order repeat prescriptions securely using 'SystmOnline' which is a web-based module provided by our computer supplier TPP. The practice also offers the facility for patients to view online, export or print any summary information from their record, relating to medications, allergies and adverse reactions.
You will need to register to use the 'SystmOnline' at the practice where you will be supplied with a username and first password (which you can then update after your first log-in). When registering you will need to provide photographic ID to verify your identity for security purposes.
